Prior to mini-scar facelift techniques, procedures required long, extensive incisions, which left visible scars. The incisions, and the scarring, have been significantly reduced with endoscopic surgery. Who is a candidate? If you have experienced droopiness of the cheek tissue causing deepening of facial grooves, loss of "cheek bone", or prominence or appearance of a "fold of fat" over your lower cheek.

A small incision is made in the scalp above the ear, another under the eyelashes of the lower eyelid (the same incision which is used for cosmetic lower eyelid surgery) and a small incision hidden inside the upper lip. The cheek tissue is then elevated to a higher location (and endoscopic midface will give a better result).

Sutures are removed within 14 days – bruising and swelling decrease to "socially acceptable" levels in 10 to 20 days with continued improvement. The intended result will give you a more youthful and rested appearance to the midface.

What Is It and Why Should You Care
The endoscope is a surgical instrument, which has been used by orthopedic surgeons for many years. It allows the surgeon to "see" inside the human body because the endoscope has a tiny camera on the end of it. When the surgeon inserts the endoscope into the body, the camera displays what the camera "sees" inside of the patient on a TV screen. The endoscope was adapted to plastic surgery just over seven years ago. Endoscopic plastic surgery is a beautiful technique and a powerful tool for the plastic surgeon who is well trained and highly experienced in the method. A small percentage of plastic surgeons are well qualified in this technique. Many plastic surgeons are unable or unwilling to pursue this highly specialized field. For those who are, it allows the plastic surgeon to handle the muscle and tissue much more gently than traditional surgical techniques. This allows for less bleeding, less swelling, and faster healing. It also enables the plastic surgeon to make much smaller scars on the patient than the older, more traditional techniques allow.

Endoscopic plastic surgery can be used to perform breast enlargements, forehead lifts, neck lifts, and cheek lifts.

To me, the gifts that the endoscope allows the plastic surgeon to give his patients are invaluable. For example, the old way of making a brow lift is to cut from the ear, over the top of the head, and down to the other ear. Who wants that? The scar is long and when the hair is wet it is not uncommon to see the scar.

An endoscopic brow lift scar is small and approximately one inch long. Typically, there are three to four in the hair-bearing scalp above the forehead and two small ones in the scalp to the side of the forehead.

Many years ago there were no options if you wanted a brow lift. Today there is this wonderful option. The healing is faster, there is less bleeding, and there is much less scarring by the endoscopic method. It is a technique for the 21st century, which was proven highly effective in the 20th century.

I called endoscopic plastic surgery a beautiful technique, and it is. Along with the brow being lifted, the patient's forehead looks beautiful and scars far less than with the traditional forehead lift. This is why you should care about endoscopic plastic surgery. Why would you want a huge scar if you could have very few tiny ones?

What a lovely procedure! If you are in your late 20s to late 30s, you may be a candidate for a mini-scar neck lift. This procedure is for patients with a poor neck/jaw line that is a result of localized deposits of fat and lax platysma (superficial neck muscle).

A relatively small incision is made under your chin and behind your earlobes. The fat is suctioned and the muscle tightened. The procedure is relatively short, and the same goes with the recovery.

Eyelid surgery incisions are made where the natural crease should be in the upper lids and below the lash line in the lower lids. Excess fat and skin are carefully removed. All sutures are usually removed within four to six days.

If you or your child has ears that stick out too far or have an undesirable shape, ear surgery can reshape your ears by creating normal folds and a more agreeable protrusion. This procedure is generally performed beginning at age five or six, but even adults can benefit. Ear surgery is performed under sedation with a local or general anesthesia, and patients return home immediately after surgery. The ears will be wrapped for several days to promote proper healing, and postoperative discomfort is minimized with medication. After a few weeks, all protective dressings are removed and you can enjoy your new look.

Full cheeks are a sign of youth. Look at the models in Vogue or Glamour magazine. Look at pictures when you were in your 20s. As we get older, our cheeks drop. The fullness over the cheek bones and under your eyelids disappear. You develop a deeper fold between the side of your nose and the outer corner of your mouth. Some people call this their "hound dog look."

An endoscopic cheek lift is a neat way to help reverse the years and restore a more youthful cheek and face. The incisions are relatively small and are located under your eyelashes. This is the same incision for doing a "blepharoplasty" or eyelid surgery. In fact, many patients combine the two (i.e. removal of the lower eyelid fat bags and elevating the cheeks) since they are done through the same incision.

This procedure is commonly performed under twilight sedation. Generally, swelling and bruising are minimal and recovery is relatively quicker.

You have probably commented or heard someone saying "doesn’t she have nice cheekbones" or "doesn’t she have a nice jaw line?" For years people have considered nicely sculpted cheekbones and a good chin as a thing of beauty. All you have to do is look at various magazines, including Vogue and Cosmopolitan, and view the models in the advertisements. A number of people are not blessed with these attributes. For those who are not and are interested in enhancing these areas on their face, there are procedures which can be performed.

Dr. Joshua Halpern has adapted some of his Endoscopic (mini scar) techniques with the placement of cheek and chin implants. The operations can be performed under twilight sedation. Patients tolerate the procedure so well that postoperative pain or discomfort are not an issue for many of the patients. The results are immediate. With the finesse techniques that Dr. Joshua Halpern utilizes, swelling and bruising is kept to a minimum. This allows the patient to return to work more quickly.

Patients have benefited from enhancing their faces where their friends have noticed that something has changed, but the change is so natural that they do not attribute it to any surgery.

Sexy pouty full lips. Many people would like to have this feature. A quick fix includes artificial fillers like collagen, Restylane®, Cymetra, and others. These are a temporary solution.

If you would like a more permanent procedure, you may be a candidate for a dermal fat graft. This is a relatively quick procedure with a fairly short recovery. Here, your own skin and fat is harvested from an old scar or hidden in a crease and placed in your lips.

RHINOPLASTY HAS EVOLVED INTO AN OUTPATIENT PROCEDURE Rhinoplasty, commonly referred to as "nose jobs," has evolved over the years along with most medical procedures. What was once a painful and laborious operation done in a hospital has become a nearly pain-free outpatient surgery. That's because experienced and trained cosmetic surgeons now work on reshaping the nose rather than attempting to completely alter the facial feature.

Rhinoplasty, or surgery of the nose, is one of the most common of all plastic surgery procedures. Rhinoplasty can accomplish various goals: it can reduce or increase the overall size of your nose; it can narrow the span of your nostrils; or it can change the angle between your nose and upper lip. Rhinoplasty is especially useful in correcting a birth defect or injury, and can also relieve certain breathing problems.

Dr. Halpern likes to first listen to the patient's goals. "I like to understand what they do not like about their nose and what changes they want." After examining the patient, Dr. Joshua Halpern makes a detailed plan for the patient. "No two noses are alike. I tailor the operation to the patient's wishes."

Rhinoplasty usually takes an hour or two, though more in depth procedures may take a little longer. During the surgery, the skin of the nose is separated from its supporting framework of bone and cartilage, which is then sculpted to the desired shape. Finally, the skin is redraped over the new framework.

With Dr. Halpern's finesse technique, recovery is quicker, with less swelling and bruising. It is not uncommon for patients to have a pain-free recovery. Dr. Halpern also has done away with the old technique of packing the patients' noses after surgery. "I don’t find it necessary, and patients do not like it."
